Diverse Community Care, based in Western Sydney, is a thriving not-for-profit organisation providing a wide range of services to frail older people, people with a disability, and their carers. They are currently seeking a Service Coordinator for their Rydalmere team.

This position is Full Time and the succesful candidate will be responsible for coordinating service access for frail older people, people with dementia, people with disabilities and their carers to appropriate support, respite and domestic assistance to enable people to remain living in their own homes in the community for as long as possible.

A tertiary qualification in the human services field and or a minimum of three years’ experience in a similar role is essential for this position.
